New Patients
We are happy to register new NHS patients within our catchment area. HOWEVER
During the Covid-19 emergency your GP practice, like the rest of the NHS, is prioritising patient care. To minimise administrative work, all GP practices locally are requesting that you (and other members of your family) do not seek to re-register with another GP practice at this time. You should only ask to register with a new GP practice if:
- You, and your family, have physically moved to a new house and are now living in a new GP practices area
- You have a new-born baby, or a newly adopted baby or child
- You are a first degree relative of a currently registered patient who has returned to live with their family
- You do not currently have a GP, and need to register with one
If you have moved to a new house, but remain within your current GPs practice area, please do not re-register with a new GP practice at the present time, even if you intend to do so in the future.
Please do not seek to register with a new GP practice unless one of these four reasons apply to you.’
How to Register
To register with our practice we need two registration forms to be completed. Please follow the links below to download the GMS1 form and our New Patient Questionnaire Form.

After you have completed these forms, print them to take it with you to the surgery. You must attend the surgery with 1 form of identification (ID) and 1 form of the proof of address for adult.
Acceptable Identification; tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc. If you do not have any proof of address please discuss with our reception team. This will not prevent your registration.
For children registration, please bring your child birth certificate and red book with you.
Until you have registered and you will not be able to make an appointment.
New Registration Health Check
Once registered, you will be asked to make an appointment with our health care assistant for a new registration health check. This appointment enables us to collect medical information before your medical records arrive from your previous practice. It is also the start of screening for other medical problems. If you medical history if particularly complex our HCA will book you a follow up appointment with a GP. Normally, you will need to complete a registration medical before seeing one of our GPs.
If you are on any regular medication please ensure you have received sufficient from your previous practice before registering as we may not be able to issue a repeat prescription immediately.
